“Automation of ENS alerts provide the health centers with real-time notifications anytime a patient arrives at an ER , admitted/transferred and/or discharged from a hospital. This allows for improved care coordination. Because of the ability to provide the technology that supports this effort, health centers have seen a 47% reduction in 30-day readmissions among ED patients, 20% reduction in 30-day readmissions among hospitalized patients, and a 33% increase in follow-ups within 30 days of hospital discharge.”

“The Payer Integration project helps health centers view attributed patients from 3 MCOs to identify their care gaps and to reach out to those not yet engaged with their primary care provider. It supports better care coordination, strengthens the patient-provider relationship, and improves data accuracy by sending supplemental clinical quality data from EHRs back to the MCOs.”
